Group content is the main exception. Groups on Facebook are almost always restricted to members, even when a group is listed as "public" in search, and Facebook does not expose that video data to an unauthenticated request — so links from inside groups will typically fail.

Quality is set entirely by how the original page or profile uploaded the video. Some pages consistently publish in HD; others, especially reposts from mobile, are only ever available in a single lower-resolution encode. There's no way to request a higher resolution than Facebook itself generated.
